LIVING WELL - A PROGRAMME FOR ADULTS WITH LONG-TERM HEALTH CONDITIONS
BACKGROUND INFORMATION
Maeve Carmody, Self-management Support Coordinator, Health Wellbeing &
Strategy, Cork Kerry Community Healthcare is inviting Expressions of
Interest (EOI) from organisations with a track record of working
effectively with communities to co-ordinate and deliver the Living
Well Chronic Disease Self-Management Programme (CDSMP) in partnership
with Cork Kerry Community Healthcare (CKCH).
Successful organisations will be contracted by HSE via Section **Apply
on the website** Grant Agreement (Health Act *)
to deliver a specified level of service. It is essential that the host
organisation for this post has a track record of working effectively
with communities to deliver programmes that improve health and
wellbeing. Previous experience in delivering the programme will be an
advantage but is not essential.
The geographical area for the service is Cork and Kerry.
Living Well is a free 6-week group-based self-management programme for
adults with long-term or chronic health conditions. It supports people
to develop practical skills and confidence so they can better manage
their chronic health condition, and make changes for a healthier life.
The programme is delivered by two trained leaders and at least one
leader must be living with a long-term health condition (peer). The
programme can be delivered in person or online. More information is
